When you walk into our Woodbridge dental office, you’ll be warmly greeted at the front desk.
New Patient Information
Please download the forms, complete your information and bring them with you to the office for your first appointment.
If you haven’t already filled out your paperwork, we’ll give you forms to complete while you wait. Please arrive approximately 10-15 minutes early if you have not already completed the necessary history forms. At this time, we’ll also collect any insurance information you have.
Once you’re in our system, one of our team members will take you back and review your medical and dental history with you. If you have any questions or concerns, be sure to mention them so we can best assist you and address your needs.

Do you take my insurance?
We accept most major insurance providers, including Cigna, Geha and MetLife. We also accept pediatric patients with Medicaid.
What should I do for a toothache or dental emergency?
If you’re experiencing any painful symptoms, call us right away. We offer same day emergency appointments. If you’re an existing patient, you can also call our office after normal business hours to speak to our on-call dentist.
